How to Fill Out Schengen Visa Application Form
This sample Schengen visa application form is based on my personal application for a short-term tourist visa to Spain. I applied as a solo traveler, single (never married), financed the trip entirely myself, and planned to stay in hotels throughout my trip rather than staying with friends or family in Spain. (Also note that I have no middle name/initial in my government documents).
For this application, I was also working as a freelancer/independent contractor without an Income Tax Return (ITR) and had previous international travel history. I applied through BLS Spain and submitted an itinerary covering Spain only, with no other Schengen countries included in my application itinerary.
The completed form below shows how I filled out the different sections and selected the applicable options based on my own circumstances. It is intended to serve as a practical reference for applicants who may have a similar profile, particularly solo travelers who are self-employed or freelance, are financing their own trip, and are staying in paid accommodations.
