In 2003, Amy Loughren is a single mother and nurse working in the intensive care unit at Parkfield Memorial Hospital in New Jersey. She is dealing with cardiomyopathy, a fact she keeps private out of concern for her job. Without health insurance, she must continue working for several more months to become eligible for coverage that would allow her to receive a needed heart transplant. The hospital brings in experienced nurse Charles Cullen to assist with night shifts, and the two quickly form a close friendship. Charlie learns of her condition and agrees to keep it confidential.

When an elderly patient named Ana Martínez, who had been under the care of both Amy and Charlie, experiences a sudden and unexplained medical decline, the hospital’s administrative board notifies the state police. Detectives Danny Baldwin and Tim Braun are assigned to look into the matter. Risk manager Linda Garran downplays the incident, stating it was unintentional and reported only to follow standard health protocols. Baldwin becomes concerned after noticing the report was filed weeks later, after the patient’s remains had already been handled. He focuses attention on Charlie and learns of a prior minor conviction from 1995. When the detectives speak with Amy, she observes that insulin appears to have been given to Martínez, shown by lowered C-peptide levels, even though the patient was not diabetic. Amy defends Charlie’s character and prefers not to become further involved.

Baldwin and Braun try to gather information from hospitals where Charlie previously worked, but receive little cooperation. Parkfield eventually shares limited findings with the police. Baldwin finds the information incomplete and confronts Garran, resulting in the detectives being barred from the hospital. Shortly afterward, another ICU patient, Kelly Anderson, suffers a seizure and an unexplained medical decline. Amy discovers that insulin had also been administered the night before. She contacts an old colleague, Lori Lucas, who had worked with Charlie at another facility. Lori describes a pattern of similar unexpected declines among patients on Charlie’s ward, with insulin found in several cases. Alarmed, Amy checks the hospital’s supply storage and finds evidence that several IV bags have been compromised. During this discovery she collapses and is taken to the emergency room.

Now convinced of Charlie’s involvement, Amy contacts the detectives. They arrange for an examination of Kelly’s remains, which shows a combination of insulin and digoxin. Meanwhile, Garran dismisses Charlie, citing minor inconsistencies in his résumé. The detectives ask Amy to meet with Charlie in the hope of learning more, but he becomes agitated when she raises the subject of his departure from the hospital.

Charlie is taken into custody, yet he does not provide a recorded statement. Amy volunteers to speak with him. During a calm conversation she encourages him to be honest. After some reflection, Charlie acknowledges his actions, simply saying he “did it.” When Amy asks why, he replies that no one had stopped him.

A closing note explains that Charlie received a lengthy prison sentence in connection with the cases of 29 patients, though the full number may have been considerably higher. Amy successfully underwent the heart procedure she needed and later settled in Florida with her daughters and grandchildren.
