Our program is designed for adults transitioning from homelessness, hospitals, rehabilitation centers, shelters, or unstable housing situations who need a structured living environment while regaining independence.

What We Provide for Social Workers
We reduce the housing search burden so social workers can focus on care.

- Full placement coordination
- Support during application and move-in

- Communication between landlord, client, and caseworker

You simply refer the client — we handle the housing steps.

Program Limitations
- We do NOT take on rent responsibility
- We do NOT guarantee payment for the resident
- We do NOT run medical or behavioral services

- We do NOT provide 24/7 staffing
Referral Requirements
Please ensure the client:
- 18+
- Has valid ID or can obtain one
- Has income (earned, SSI, SSDI, pension, or program)
- Can sign their own lease
- Does not require medical supervision
- Understands and agrees to basic housing rules
- Has a caseworker or point-of-contact
