How to Complete Your Client Questionnaire

Thank you for choosing the Intentional Home Shopping Experience by Hadassah Ruth Sacred Home Healing.

Please complete the questionnaire as thoroughly as possible. The more information you provide, the better I can create a personalized shopping plan tailored to your style, home, and budget.

Option 1: Print & Complete by Hand (Easiest)

1. Download and print the PDF questionnaire.
2. Fill out each page by hand.
3. Once completed, take clear photos or screenshots of each page.
4. Reply to your confirmation email and attach all completed pages.

Option 2: Complete the PDF Digitally

1. Download the PDF to your computer or tablet.
2. Open the file in a PDF reader such as Adobe Acrobat Reader or Preview (Mac).
3. If available, turn on the Show Form Filling Toolbar to type directly into the form.
4. If the form fields are not fillable, use the Markup or Comment toolbar and select the Text Box tool to type your answers anywhere on the page.
5. Save the completed PDF before closing the document.
6. Reply to your confirmation email and attach the completed PDF.

Take several well-lit photos of the room from each corner so I can clearly see the entire space.

Video Walkthrough (Recommended)

A short video walkthrough helps me better understand your room's layout, lighting, and overall flow.

Room Sketch & Measurements

Please include a simple hand-drawn sketch showing:

• Overall room dimensions
• Wall lengths
• Window locations
• Door locations
• Ceiling height (if known)
• Measurements of any furniture you plan to keep

Your sketch doesn't need to be perfect—a simple drawing is all I need.

Inspiration Photos

Please attach 5–15 inspiration images (Pinterest pins, screenshots, magazine photos, etc.) that reflect the style and feeling you'd like to create.
