Purpose of This Tracker
This tracker is designed to help student-athletes, families, coaches, and educators monitor academic performance, maintain eligibility, improve literacy skills, and prepare for life beyond high school athletics.

How to Use Each Form
1. Academic Eligibility Checklist
Use this form to:
- Track progress toward GPA and course completion standards
- Document reading level and intervention status
- Identify academic areas that may affect eligibility
Suggested Frequency: Update each grading period or semester
2. Weekly Progress Tracker
Use this form to:
- Check off completion of reading logs, homework, and academic behaviors
- Set weekly reading fluency or comprehension goals
- Write brief notes on successes or concerns
Suggested Frequency: Complete at the end of each week
3. Athletic Participation Log
Use this form to:
- Document all sports-related time commitments (games, practices, travel)
- Reflect on how athletics affect academic availability or performance
Suggested Frequency: Update weekly or after each event
4. Parent & Coach Communication Log
Use this form to:
- Record who you spoke with, when, and what was discussed
- Keep track of concerns, questions, and decisions for both academics and sports
Suggested Frequency: Fill out after each conversation or meeting
5. Post-secondary Readiness Reflection
Use this form to:
- Assess if the student meets NCAA eligibility standards
- Reflect on reading ability, academic performance, and future plans
- Identify a backup career or education path in case athletics are no longer viable
Suggested Frequency: Review quarterly or during IEP/transition meetings
Tips for Best Use
- Store all completed forms in a binder or shared digital folder
- Review with a coach, counselor, or parent at regular intervals
- Use data from the forms in IEP meetings, parent conferences, or goal-setting sessions
- Encourage the student to participate in updating and reflecting
