Manager’s Appraisal Form

A manager’s appraisal form is a formal governance document used to assess, evidence, and review the performance, competencies, leadership behaviours, and regulatory compliance of a care manager over a defined period (usually annually or bi‑annually). It provides a structured framework for evaluating how effectively the manager leads the service, maintains CQC‑aligned standards, and ensures safe, high‑quality care.

Purpose and Impact

The manager’s appraisal form ensures:

  • Consistent, fair, and transparent evaluation
  • Clear evidence of compliance for inspections
  • Strong governance and accountability
  • Improved leadership capability
  • Better outcomes for people using the service
  • A structured pathway for professional growth

Core Components Of The Manager’s Appraisal Form

  • Role Responsibilities — A clear outline of the manager’s duties, including leadership, governance, compliance, staffing, safeguarding, and operational oversight.
  • Leadership & Management — Evaluation of leadership style, decision‑making, delegation, communication, and ability to motivate and develop staff.
  • Regulatory Compliance — Assessment of adherence to CQC regulations, audit outcomes, incident management, reporting, and quality assurance processes.
  • Quality of Care Delivery — Review of how the manager ensures safe, person‑centred care, monitors outcomes, and responds to feedback or complaints.
  • Safeguarding Performance — Evidence of safeguarding leadership, risk management, incident handling, and compliance with local authority procedures.
  • Workforce Management — Evaluation of recruitment, supervision, training compliance, rota management, and staff wellbeing.
  • Operational & Business Performance — Review of financial stewardship, resource management, service growth, and business continuity planning.
  • Professional Development — Identification of training needs, CPD progress, leadership development goals, and future objectives.
