A puzzle with the demand for a longevity intervention is that everyone has an opinion and almost nobody has read the trial registry. This PDF is the registry read, laid out one intervention per page: rapamycin, metformin, senolytics, NAD+ precursors, caloric restriction, young-donor plasma, GLP-1-adjacent compounds, and 15 others that have entered at least one human trial search on ClinicalTrials.gov.

Twenty-two sections, each with the same fields: the mechanism it targets, how far the human evidence has actually gone (descriptive registry facts, animal-only, small early-phase human trial, or a full randomized controlled trial), what direction the referenced result pointed, the pivotal trial's registry number and sample size, and every primary source behind that row, counter-evidence included.

Counter-evidence stays in the table on purpose. A source that reports a null result, an adverse effect, or a result that runs the other way in a different model sits right next to the paper reporting benefit. A table that only lists the papers agreeing with the headline is not evidence, it is marketing, and that is exactly the problem this ledger is trying to fix.

What you will not find here: a recommended dose, a supplement stack, or a should. This is a research-literature summary, not medical advice, and it says so on every page. Where a compound has no registered human study at all, the ledger says that plainly instead of filling the gap with a mouse result dressed up as a human finding.

In the PDF: - 22 interventions, one section each, in the order rapamycin through partial epigenetic reprogramming - Evidence level and result direction for every row, defined once at the front so the labels mean the same thing in every section - The pivotal registered trial for each intervention where one exists, with its ClinicalTrials.gov number, sample size, and status - 70+ primary-source citations with journal, year, PMID or DOI, and a direct quote from the paper - A short note on how the ledger itself is checked before publishing: every quote is verified against the cached source text, and the checker is run against fourteen deliberately broken test rows first, to confirm it actually catches the kind of error a reader would notice

This edition is v2026Q3, data cutoff 2026-07-27. The free companion page has the short version of the same table; this PDF is the one with the full citation list per intervention for offline reading.

Please read before buying: this is a reference document, not software and not a subscription. It will not tell you what to take. If new registered trial results change a row materially, the next quarterly edition covers it; this purchase is for the current edition.
