Model Permissions Form

When I go to a shoot, I want to make sure I know what I am and am not allowed to do with images I get back.  I also want to make sure that I have all of the information for the photographer and know how they felt about the shoot.  This form does it all!  Asks for permission to submit to publications, post online and more!
