This is a compilation of things I’ve learned in my 3 years of training, adapted for your use. It makes writing notes on shift much faster, and it definitely makes me a better doctor and document-er. 

On my blog you will find some resources about documentation tidbits for residents/young attendings, and you should always strive to write notes that explain why or why not, including appropriately documenting your history, physical exam and workup.
