Transition Survey

This transition survey is meant to be used as an informal tool to get information needed for these three elements of special education paperwork: an informal assessment as part of a three-year reevaluation and these two parts within the Individualized Education Program (IEP): the Present Levels of Academic Achievement and Functional Performance (PLAAFP) section and the Transition Goals section.

There are 2 ways to use this survey: the student can fill it out independently, OR this can be used as a note-taking guide for an oral conversation with a student.  Either way, answers to these questions will provide enough information for the current skills and goals for the evaluation report and IEP.
