What is a Patriot Act disclosure form for?

It is a form to help the government fight the funding of terrorism and money laundering activities, federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account.

To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies every customer. When applying for a loan, applicants will be asked for their name, address, date of birth, and other information that will allow lenders to identify them. Applicants will also be asked to show their driver's license or other identifying documents. So, you as the notary public ( loan signing agent) are responsible for making sure that this form is filled out correctly. With this download, I have provided you with two great examples of what this form could look like. Now forms vary from signing agency to signing agency but these are two of the most generic forms that you will encounter while doing a loan signing.

This information is just a few examples of the huge library of documents and knowledge that needs to be taught to new notaries. Please stay tuned For more information as our library grows every month. This information will help you greatly. God bless
