Preparing an EHC needs assessment request can involve gathering information from several different people, documents and systems. The EHCNA Evidence Checklist helps you keep track of what evidence you have, what is still missing and who is responsible for providing it.
The editable checklist includes space to record key pupil details, link directly to an evidence folder and monitor progress as supporting information is collected.
It prompts you to consider evidence such as:
- examples of the pupil’s work
- aided and unaided work
- the history of SEN Support through the Assess, Plan, Do, Review cycle
- notes from relevant review meetings
- attainment information
- relevant standardised assessments
- pupil views
- parent or carer views
- attendance information where relevant
Each item can be assigned to a member of staff, given a progress status and accompanied by notes or links, helping you keep everything together rather than chasing evidence at the last minute.
