Digital Transition Assessment Pack: Fillable PowerPoint and Team Forms, High School
This digital transition assessment pack gives you five domain skill assessments as fillable PowerPoint files, plus staff and family input forms, and a printable Team Summary Sheet you tally by hand.
Why I made this: Gathering transition data from the student, teachers, job coaches, and family used to mean a stack of paper and a long night of tallying. I wanted all three views in one place, lined up next to each other so the IEP team can see where they agree.
What's included:
• A Student Self Assessment PowerPoint: 5 domains, 12 skills each (60 skills), with a goal box for every domain
• A Staff Input PowerPoint rating the same 60 skills
• A Family Input PowerPoint with 30 key skills and questions about hopes for life after high school
• A Team Summary Sheet (printable PowerPoint pages) to tally each domain, list priority skills, and note where views agree or differ
• A teacher guide PDF with setup, pacing, accommodations, and a scoring guide
How to use it: Open the files in PowerPoint or Google Slides and assign a copy to each person. Students finish in two short sessions. After staff and family return their forms, tally on the Team Summary Sheet and use it to write present levels, goals, and transition activities.
Who it's for: special education teachers, transition coordinators, and case managers writing transition plans for students in grades 9 to 12 and 18 to 21 programs.
Format: Four PowerPoint files (.pptx) plus a teacher guide PDF. Open them in PowerPoint, or upload them to Google Drive and open them with Google Slides. The interest inventory is not included here. It is sold separately as the Digital Transition Interest Inventory.
