HCBS Waivers Mini-Guide: English and Spanish for Transition Families, Ages 14 to 21
This HCBS waivers mini guide explains, in English and Spanish side by side, what home and community based services are, why waitlists matter, how aging out of school connects to adult services, and questions to ask.
Why I made this: Waiver services can take years to open. Families who wait until exit often learn that too late. I wanted a bilingual plain guide so Spanish speaking families can start asking the right questions while their student is still in school.
What's included:
• Cover and how to page
• What are HCBS waivers
• Why waitlists matter
• Aging out of school and adult services
• A simple timeline by age
• Who to call and what to ask
• A family planning checklist
• Words to know and local contacts
How I use it: Share it at transition meetings from about age 14 on. Fill in your state disability services office. Use the timeline page to plan when to apply.
Who it's for: families of students ages 14 to 22 who are more comfortable reading in Spanish, and the transition teachers, case managers, and family liaisons who work with them. English and Spanish sit side by side, so bilingual families and staff can read together.
Format: 9 printable pages, US Letter, English and Spanish side by side on every page. General information, not legal or benefits advice. Rules vary by state.
Prefer English only?
Compared with the English version
The same:
• Same topic as HCBS Waivers Mini Guide, written in plain language for families.
Different:
• English and Spanish side by side on every page, so Spanish speaking families and English speaking staff can read together.
• Uses common US Spanish terms for school and benefits (for example preparatoria and su hijo o hija). IEP stays as IEP.
• Written for this edition rather than copied page for page.
