12 week year planner - Notion template

Run your next 12 weeks like they are your whole year.

The 12 Week Year method breaks big ambitions into short, focused execution cycles. This template gives that method a real home in Notion, fully built and ready to use from day one.

What is inside

Seven connected databases, each with a clear job:

• Weeks – the backbone of your cycle, with automatic completion tracking for every week
• Goals – the handful of outcomes that matter this cycle, with live progress percentages
• Tasks – the weekly actions that move each goal forward, with overdue flags built in
• Milestones – measurable checkpoints for each goal, calculated automatically as you update your numbers
• Habits – the behaviors you want to build or maintain. Habit Log is your daily check in.
• Weekly Reviews – a short structured reflection at the end of each week, with your numbers already filled in for you

Why it actually works

Everything is wired together with two way relations, rollups, and formulas, so progress flows upward on its own. Finish a task and your week's completion percent updates. Log a habit and your weekly totals update. Update a milestone and your goal's progress bar updates. No manual recalculating, ever.

Stop planning your year in vague resolutions. Break it into twelve weeks, track it properly, and actually finish what you start.
