Consequence Mapping: Behavior & Decision-Making Worksheet
Help clients slow down, examine what happened, and explore how a different choice could lead to a different outcome.
Consequence Mapping is a simple visual worksheet designed to help clients examine a recent situation by tracing the connection between what they wanted, what they did, what happened next, and what they could have done differently.
The worksheet guides clients through five questions:
• What did I want?
• What did I do?
• What happened?
• What could I have done instead?
• What would have happened then?
The road-map format makes the exercise easy to follow and can help turn an abstract discussion about behavior and consequences into a concrete, visual conversation.
Useful for exploring: impulsive decisions, emotional reactions, interpersonal conflict, problem-solving, behavioral patterns, and alternative responses.
Designed for: Mental health professionals, counselors, social workers, behavioral health providers, and other helping professionals.
Format: 1-page downloadable PDF worksheet.
