A few years ago, I had a series of health check-ups. Every doctor's office I went to made me fill out an intake form. Each time, I had to recreate the list: my health history, family history, appointment dates, test results, and medications. It was super annoying to have to recreate the same list of information every time. How many times can I try to remember which grandma had glaucoma or if that odd test result was two or three years ago?
It's the type of information that lives in a gray area: I don't need it often enough that it's obvious I need to keep a record of it all, but I need it often enough to make it super annoying to have to recreate the list every time I need it.
Try as I might, I'd forget the dates of past appointments or not remember precisely how a diagnosis or treatment plan was phrased. And since I’ve changed doctors and health plans over the years, there’s inconsistent communication among doctors, facilities, and insurance providers, which can make tracking down that information challenging.
