This comprehensive health assessment is required by the Texas Administrative Code 553.259 (b). The comprehensive health assessment should be the tool that gives a synopsis of the residents’ health, life, needs, desires, and concerns.
Comprehensive health assessments must include:
- the location from which the resident was admitted
- primary language
- sleep-cycle issues
- behavioral symptoms
- psychosocial issues
- Alzheimer's/dementia history
- activities of daily living patterns
- involvement patterns and preferred activity pursuits
- cognitive skills for daily decision-making
- communication ability
- physical functioning (transfer status; ambulation status; toilet use; personal hygiene; ability to dress, feed and groom self
- continence status
- nutritional status (weight changes, nutritional problems, or approaches)
- oral/dental status
- diagnoses
- medications (administered, supervised, self-administered)
- health conditions and possible medication side effects
- special treatments and procedures
- hospital admissions within the past six months or since last assessment
- preventive health needs
- TB testing (best practices)
Assessment must:
- be completed within 14 days of admission, annual renewal, or resident change of condition by the owner and/or manager of the facility
- notify residents or legally authorized representatives of updates to resident information they did not provide
- notify residents and/or legally authorized representative prior to admission, annually, change of condition of Comprehensive Assessment
