Requirements

Microsoft Excel 365 or Excel 2021 or later. The default collection route uses a free form on your organisation's own Microsoft or Google account; no other software is required, and the included slips route needs nothing beyond a printer.

Licence

Licensed for internal use by a single organisation. Federated organisations — multi-academy trusts, group subsidiaries, integrated care systems — require a separate licence per governable entity. Consultancy or client-facing use requires a separate practitioner licence.
