Extract / unzip the folder before opening the files.

Open 00-START-HERE-core-patient-record.html first.

Use the all-files dropdown at the top of each page to move around the product.

Open 02-core-patient-record.html to complete the fillable record, including the “completed by” and “record last reviewed / updated” fields where helpful.

Use the save button to save entries on the device you are using.

Use print if you want a paper copy.
