OASIS for MRCOG Part 2 and 3: classify it, repair it, and answer the question on it
Built from RCOG Green-top Guideline No. 29, the 2023 RCOG pelvic floor policy position and the OASI Care Bundle study, with a table of what has changed since 2015.
### The problem
Perineal tear questions look easy until the stem says "she is unsure whether more or less than half of the muscle is torn." Then the grade, the suture, the technique and the follow-up all depend on details you have to hold in your head at once.
GTG 29 dates from 2015. Its review is overdue, an update is in development, and in the meantime the OASI Care Bundle and the 2023 pelvic floor policy position have changed how prevention is taught. Notes written from the guideline alone miss that. Notes written from memory miss the numbers.
### What this is
A single revision module on OASIS, in two files that work together.
**The study notes** run to 23 pages in 11 sections: classification, pathophysiology, risk factors and prevention, identification, repair, postoperative care and complications, follow-up and future delivery, risk management and audit, the Care Bundle, an evidence-gap table and an exam-traps section with a numbers cheat sheet and self-test. There are six original diagrams, including the classification matrix and a step-by-step repair algorithm.
**The question bank** has 70 items: 20 single best answers, 34 extended matching stems in 5 sets, and 16 sub-questions across 4 clinical vignettes. Every answer and explanation sits directly under its question, so there is no key to flip to. Each explanation cites the guideline by name, year and section.
### What you get
- 23-page study notes (PDF and Word)
- 6 original diagrams: classification matrix, risk-factor chart, prevention evidence map, repair algorithm, follow-up pathway, Care Bundle results
- Evidence-gap table: each 2015 recommendation against what has changed
- Numbers cheat sheet and self-test
- 70-item question bank (PDF and Word) with inline answers
- Sources named in the file: RCOG GTG No. 29 (2015), RCOG pelvic floor health policy position (2023), OASI Care Bundle study (BJOG 2020)
### What you will be able to do afterwards
- Grade a tear from a vignette and say why 3a, 3b and 3c are different
- Recite the repair sequence in order, including what happens to the internal sphincter and the mucosa
- Quote the numbers that matter: the warm compress relative risk of 0.48, the 5 to 7% recurrence risk, the 6 to 12 week review
- Explain why a rising OASIS rate is not automatically poor care
- Say what the guideline recommends, and what has changed since it was written
### Who it is for
MRCOG Part 2 and Part 3 candidates who revise from RCOG material and want one topic done properly. It also suits registrars preparing to teach or audit OASIS on their unit.
### Why it is different
- **Written by a practising OB/GYN**, not by a question-writing service.
- **Every vignette and distractor is original.** Nothing is copied or adapted from an existing question bank, and the explanations paraphrase the guideline rather than quote it.
- **The currency check is part of the product.** The notes state plainly that GTG 29 is still current, that its review is overdue, and where a point rests on a source outside the guideline.
- **Explanations carry the reasoning.** You find out why the wrong options are wrong, which is what transfers when the exam rephrases the stem.
This describes how the product was built. It is not a promise about your exam result.

This module is a study aid built from RCOG Green-top Guideline No. 29 (2015), the RCOG pelvic floor health policy position (2023) and one published quality-improvement study. It does not replace the RCOG curriculum, Green-top Guidelines or official RCOG question banks. It does not guarantee exam success. Guidelines are revised on an ongoing basis, and an update to GTG 29 is in development. Always verify against current RCOG guidance.
