Catch the Pattern | Thoughts, Feelings & Actions Worksheet
Help students recognize the connections between what happens, what they tell themselves, how they feel, and what they do next. “Catch the Pattern” uses a simple visual loop to guide students through an experience with curiosity and without self-blame.
Students can map out a situation and identify a place where they could pause, question a thought, or try a different action.
Students will explore:
A situation they experienced.
The thoughts they had about what happened.
The feelings that showed up.
What they did or felt an urge to do.
One way they could interrupt the pattern.
What’s included: One black-and-white worksheet featuring a four-part visual cycle, guided prompts, writing spaces, and three checkbox options for interrupting the loop.
Use with middle and high school students during individual counseling, small groups, or social-emotional learning activities. It provides a clear starting point for conversations about recurring patterns, self-talk, conflict, and making thoughtful choices.
