UK Edition: EHCP Preparing for Adulthood Outcomes Self-Check: Year 9 Plus Annual Review Audit, SEND KS4

A quick audit to run on an EHCP before a Year 9 or later annual review. It asks 15 questions about the young person's views, needs, outcomes and provision, and whether the plan is pointing towards adult life.

Score each check as yes, partly or not yet, then turn the gaps into an action plan for the review. A one page guide to the four Preparing for Adulthood areas is included for staff and families who are new to them.

It works for one plan or as a whole caseload check across a department. 4 A4 pages.
