CLIA Waiver Application

If your having a difficult time not knowing what testing to put on your applications. We will fill out your CLIA Waiver application and submit to your state. Some states are easier than others and will require more work and paperwork. I always say give it at least 3 weeks for a approval response.

FYI: CMS-116 form is just a sample preview of what the application looks like. We will fill in all your information and have you review and sign and then submit to the state.
