Monthly Blood Pressure Log – Printable & Fillable PDFs | US Letter
Keep your home blood pressure readings together in one clear, organized monthly record. This monthly blood pressure tracker provides space for morning and evening sessions, two readings per session, pulse, and notes to discuss with your healthcare professional.
Your purchase includes two versions of the same log:
• Printable PDF: Print the blank pages and record your readings by hand.
• Fillable PDF: Type your readings into interactive fields using a compatible PDF reader on your computer. You can save your completed log and print it whenever needed. Some browser PDF viewers also support filling forms, but compatibility varies.
Each version contains four pages:
• A practical instruction page with measurement reminders and space for your personal action plan.
• An intentionally blank second page for double-sided printing.
• Two monthly log pages covering days 1–31, with morning and evening entries.
• Fields for your name, month/year, clinician-set target, actual measurement time, blood pressure readings, pulse, and notes.
• An appointment-review section for questions, patterns, and agreed next steps.
Paper size: US Letter, 8.5 × 11 inches.
Undated and reusable: Print a fresh log or save a new digital copy each month. Cross out or leave unused dates blank in shorter months. Follow your clinician’s recommended measurement schedule.
Using the fillable version: Download the PDF and open it in a compatible PDF reader. Enter your information, save your completed copy, and print it if desired. Keep the original blank file for future months. Neither version performs automatic calculations.
Double-sided printing: Choose Actual Size / 100%, flip on the long edge, and disable “skip blank pages.” The instructions will have a blank reverse, and the two log pages will print on opposite sides of the second sheet.
Digital download only. You receive two PDF files; no physical item or blood pressure monitor is included.
For adult home recordkeeping. This log does not diagnose conditions or replace professional medical advice. Personal and household use only; redistribution or resale of either PDF is not permitted.
