FBA Interview Forms for Teachers, Parents and Students: Functional Behavior, High School
You learn more about a behavior in a ten minute conversation than in a week of guessing. The trick is asking the right questions in words that people can answer. These forms do that for the teacher, the family, and the student.
What's included:
• Teacher and staff interview with plain questions: what it looks like on video, when it happens, what happens right before, what adults usually do
• Parent or caregiver interview that asks about home, sleep, routines, and what works there
• Younger student interview with simple, kid friendly questions
• Older student interview that treats the teen like the expert on their own day
• Notes columns on every form
How I'd use it:
Use these alongside observation and data, never instead of them. I like to do the family interview by phone and type the answers in as we talk. The student interview is the one teams skip, and it is often where the best idea comes from. Bring all four to the planning meeting. Editable Word file included.
Each form fits on a page or two, with plenty of room to write. The questions are open on purpose. "What usually happens right before?" gets you much further than a checklist of antecedents nobody understands. Keep your own opinions out of it until all four interviews are in, because the family and the student often see a different pattern than the staff.
Who it's for: Teachers, case managers, and school psychologists. Grades K 12.
What you download: one printable PDF, 5 pages, US Letter, plus an editable Word file so you can type in it.
