A quick way to score the Conners-3 Short Form. All you have to do is enter in the raw scores from each of the raters and the easy to use excel form will automatically fill in the Raw scores and T scores for clients. Included is one spreadsheet with a tab for each rater (self-report, parent, and teacher). The self-report contains 39 questions (7 scales), the teacher report contains 39 questions (7 scales) and the parent report contains 43 questions (8 scales). Just enter in the client responses in the yellow cells and the raw scores and t scores will be automatically calculated. This autoscoring template uses American norms. The template should only be used by mental health and medical professionals with at least some experience scoring the Conners 3 short form.
