Safety Forms & Log Kit for Alberta Trades: FLHA, Inspections, Incident Report and Safety Log (Fillable PDF, Word, Excel)
Clients and prime contractors keep asking for paperwork, and a thick manual on the office shelf doesn't help the crew at 7 a.m. This kit gives you the everyday forms in two formats (fillable PDF to print or fill on screen, and Word to edit) plus a Safety Log workbook that keeps your written hazard assessment and adds up every form you complete, month by month.
Every form is original wording, letter size and black-and-white friendly. Each one has a header block for your company name and logo, and Alberta OHS references cited by section number in plain language. The incident report includes a "Does this need to be reported?" box covering the Alberta OHS phone-in list, potentially serious incidents and the WCB 72-hour employer report. The filing guide sets out how long to keep each record type under current Alberta law.

- Toolbox talk / safety meeting record: topics, concerns raised and answered, action items and attendance. It also works as health and safety rep or committee minutes.
- 8 inspection checklists with sign-off: weekly site, shop, mobile equipment pre-use, portable ladder, harness and lanyard pre-use, fire extinguisher, first aid kit and work vehicle.
- Incident and near-miss report in 3 parts: a same-shift quick report, a supervisor investigation with contributing factors and a "why?" chain, and corrective actions with sign-off. The reporting box includes the Alberta OHS and WCB-Alberta phone numbers.
- First aid and injury record with a confidentiality note (OHS Code ss.183–184), and a 1-page site emergency plan covering likely emergencies, contacts, muster point, first aiders and the transport plan for an injured worker (Code ss.115–117 and s.180).
- Safety Log workbook: Setup (workers and ticket expiry dates), Hazard Assessment register (task → hazard → risk rating before and after controls), FLHA Log, Meetings, Talk Planner, Inspection Schedule and Log with next-due and overdue flags, Incidents, First Aid Record, Corrective Actions (open, overdue, closed), Monthly Summary for review and sign-off, and one-page print versions of the two widest tabs.
- Retention by record type (Retention tab and SSS-00): incident investigation reports at least 2 years (OHS Act s.33(7)), first aid records at least 3 years (OHS Code s.183(3)), and a suggested 3 years where the law sets no period. Each log row gets a "keep until" date you can override.
Who it's for
Small Alberta and Western Canada contractors and fab shops (roughly 3 to 20 people) whose clients or prime contractors ask for safety paperwork, or who are building the records a health and safety program needs.
COR and SECOR
These templates can help you build and keep the day-to-day records a health and safety program needs, the kind of records COR and SECOR evaluations look at. Buying or using them doesn't give you COR or SECOR. Certification comes only through a Certifying Partner's audit or evaluation. In Alberta construction, many employers use the Alberta Construction Safety Association (ACSA) as their Certifying Partner. Bid Sheets isn't affiliated with or endorsed by ACSA or the Government of Alberta.
FAQ
Will this get me COR or SECOR? No. Only a Certifying Partner's audit or evaluation does that. The kit helps you keep the day-to-day records those evaluations look at.
Is this a full health and safety program or manual? No. It's the forms and the log that a program uses every day. You still need your own hazard assessment and program, and the Safety Log has a tab for the hazard assessment.
Are these OSHA forms? No. References are to Alberta's OHS Act and Code, cited by section number in plain language. No legislation text is copied.
I work outside Alberta. Will it help? The forms are general, but the legal references and reporting box are Alberta's. Check your own province's rules.
Does it include pre-written toolbox talks? No. It includes the meeting record and a topic planner. You bring the talk.
Can I add my logo? Yes. Every form has a logo box and company name field. Use the Word files to change wording.
Can I fill the PDFs on my phone? They're fillable PDFs built to work in a free PDF reader.
How long do I keep these records? It depends on the record type. The filing guide and the Safety Log's Retention tab list the current Alberta minimums.
Can I get a refund? It's a digital download, so there are no refunds once it's delivered. Email me if any file gives you trouble.
Disclaimer
These templates are general examples for small Alberta and Western Canada employers. They are not legal advice and are not a substitute for a site-specific health and safety program, a hazard assessment of your own work, manufacturer instructions, or review by a competent person. Your employer, prime contractor and supervisors stay responsible for meeting the OHS Act, Regulation and Code and any other law that applies. Legislation references are plain-language summaries, not the law itself. Check the current legislation before relying on them, and adapt every template to your work, your equipment and your site.

Licence
One-business licence: use it across your business (all its sites and crews) on as many jobs as you like. Please don't resell, share or publish the blank templates outside your business. Completed job files and filled-in forms can go to your clients, prime contractors, auditors and regulators.
Refund policy
Because these are digital downloads, I don't offer refunds once a file has been delivered. Please check the compatibility list above before you buy, and grab the free Bid Go/No-Go Scorer to test on your computer first. If a file won't open or something isn't working, email me at support@bidsheets.ca and I'll help you sort it out.
Updates
Buyers get the 2027 update free. I'll email you the new version when it's ready. Each January I refresh the payroll rates (CPP, EI, WCB) in the estimating workbook and fab calculator, re-check the Alberta OHS and prompt-payment references in the safety and billing products, and re-check the standards references in the takeoffs.
