Three Steps From the Door, I Fell Face-Down — and My Own Daughter Couldn't Tell the ER My Medical History
I'm the person people come to when something needs to be organized. But when it was my turn to answer, I didn't have the answers — and neither did the daughter standing next to me.
For years, I've kept a health binder — the very system inside this workbook. I bring it to every regular checkup. My doctors know it. It's part of my routine.
But last Monday reminded me why "routine" isn't the same as "ready."
It was a Monday afternoon, right after work. My son-in-law was waiting to pick me up, and I was walking toward HR Door 13 to meet him — still about 20 meters from the door.
My right shoe caught on something. I couldn't stop myself in time, and I went down face-first.
A male employee nearby rushed over to help me up. At the time, I only felt pain in my right knee, elbow, and wrist — I was given a hand support bandage, some pain relief roll-on, and a cold/hot compress. It wasn't until Wednesday, two days later, that I noticed the bruises had spread across my right arm, shoulder, and knee.
By the time I got to the ER, my hand hurt too much to fill out the intake forms myself, so my daughter stepped in to help. She's a Speech and Language Pathologist and a Clinical Manager at a major rehab center — she works in healthcare. But she's lived in the U.S. for 22 years, and she realized, right there in the ER, that she didn't actually know my full medical history.
When the nurse asked about my major surgeries, I could only think to mention the most recent one — my lumpectomy. It wasn't until later that I remembered I'd also had an oophorectomy, an appendectomy, and a repair of the meniscus in my left knee — none of which my own daughter knew about.
They X-rayed my knee and my hand. Thankfully, no fractures. And in the chaos, I'd even lost my hearing aid — I only found it later, still tucked in a pocket of my backpack, using the "Find My" app.
My binder was at home. It always went with me to appointments — never to work. And on the one afternoon I actually needed it, my own daughter, a healthcare professional herself, couldn't answer for me either.
That's when I understood the difference between a good system and a complete one. It's not enough to organize your records for the visits you plan for. You have to be ready for the ones you don't.
That fall didn't inspire this workbook — I had already built it, page by page, from twelve years as my husband's caregiver and my own experience as a breast cancer patient. But that Monday tested it in a way no planning session ever could. And it showed me exactly where even I still had gaps.
When it matters most, memory is not enough — and neither is a binder that only travels to the appointments you saw coming.
Introducing: My Health Journey
Personal Medical Records and Care Planning System
A 77-page guided workbook that gives you one place — not five drawers, three apps, and a shoebox of old papers — to keep everything a doctor, a caregiver, or an emergency room might ever ask you.
Built by someone who has been the caregiver, the patient — and, on one very ordinary Monday, the person whose own system wasn't in her hands when she needed it most. Not a template made by a stranger. A system built from twelve years of hospital hallways, tested by real life, and refined because of it.
What's Inside (77 Pages, 12 Chapters)
- Your emergency-ready profile — the exact information paramedics and ER staff ask for first
- Complete medical history tracker — conditions, surgeries, allergies, family history, immunizations
- Healthcare team directory — every doctor, specialist, pharmacy, and lab in one place
- Medication & treatment management — dosages, changes, side effects, refill tracking
- Lab & diagnostic records — so a decade of test results doesn't live in scattered portals
- 5 condition-specific trackers — Cancer, Diabetes, Heart Health, Respiratory Health, and Bone Health
- Healthcare expense & insurance organizer
- Advance care planning section — because your family shouldn't have to guess
- Caregiver support network — so you're never navigating this alone
- Emergency & hospital-readiness checklist
- Annual health review — because your health journey deserves a look back, not just constant crisis mode
Who This Is For
- Anyone managing a chronic condition — or caring for someone who is
- Cancer patients and survivors who are tired of repeating their story to every new provider
- Adult children helping aging parents keep their records straight
- Anyone who has ever sat in a waiting room and thought, "I should really have this written down somewhere."
Why This Isn't Just Another Template
Most planners like this are made by designers who have never sat in an oncology waiting room. This one was built page by page, from real experience — as a caregiver for twelve years, and later, as a patient myself.
Every checklist in here exists because I needed it and didn't have it.

For many years, she helped organize medical information for loved ones. Later, during her own breast cancer journey, she experienced firsthand how important it is to have essential health records, medication lists, test results, and emergency information organized in one place.
Those experiences inspired her to create My Health Journey—a practical workbook designed to help individuals and families stay organized, prepared, and confident when navigating healthcare.
Today, through Teacher Linda Practical Guides, she continues to create practical digital resources that simplify everyday challenges and help people make informed decisions with greater confidence.
