You have the prompt level data. What eats the time is turning it into a fading plan a job coach and the IEP team can follow. I built this prompt for my own 18 to 21 and work based learning sites. You name the site type, the student in general terms, the tasks, the recent independence data, how much coach support you have now, the end target and timeline, and the safety or support constraints. The AI tool your district allows gives back a full first draft fading plan: a snapshot, a baseline summary, a fade ladder with criteria to move up, task by task notes, a hold/step back plan, a data schedule, and TEAM INPUT NEEDED flags.

What the prompt gives you back (8 labeled sections):

• A PII check, so the AI flags anything identifying before it writes

• A snapshot of current coach support vs the end target and timeline

• A prompt / independence baseline summary from the data you gave

• A fade ladder table: Level | Coach presence / prompt level | Criteria to move up | Natural supports in place

• Task by task fade notes, including what must not fade yet

• What to do if the student regresses or a safety issue appears (hold / step back)

• A data and review schedule

• TEAM INPUT NEEDED flags for staffing, site buy in, and missing facts

What's inside:

• The copy ready prompt, as a PDF page and a plain text file that copies cleanly

• How to use it with ChatGPT, Claude, Grok, or Gemini

• A NO STUDENT PII guide with quick de identify swaps

• A Fading Plan Worksheet that matches the prompt's 7 brackets

• A Weak vs. Strong input guide

• 2 real sample outputs from Claude (fictional placements, unedited): grocery bagging 18 to 21 and school cafeteria dish room grade 11

• A review checklist to use before anyone changes job coach support

Great for:

• Job coaches and 18 to 21 staff who already have prompt level data and need a clear fade ladder

• Work based learning coordinators who want natural supports before cutting coach time

• Transition teams who need honest "hold / step back" language when safety or thin supports say not yet

Please note: never enter student names or identifying information. Follow your district's AI, work based learning, staffing, and data privacy policies. Outputs are drafts for the IEP team. This is not legal advice. Fading job coach support is instructional practice and best practice; no federal rule requires a fading plan. The IEP team decides services. This fading plan is NOT an IEP, NOT a decision on services or LRE, and NOT legal advice. It does not replace your district's data forms or observation tools. Pair it with blank Task Analysis Data Sheets if you still need to collect the prompt data first.
