Log every stoppage, and the log works out the minutes and labour hours lost, even for stoppages that run past midnight. The Pareto tab ranks your causes automatically, so you can see the vital few behind most of your lost time. Filter by date, shift or area, and edit the areas and causes to match your site.

Gemba walk observation sheet (printable, fillable PDF)

Go to the real place and record what you see. Log observations against the eight wastes, capture what the team tells you, note anything unsafe and finish with your top three opportunities. Includes a one-page guide to walking the gemba.
