This self-completion Advance Care Directive — often called a Living Will — lets you record your own health care wishes directly, in your own words. It is a personal statement of your values, beliefs, and treatment preferences that guides your family, loved ones, and health care team through the hardest decisions they may ever face on your behalf.

Prepared by Solvine Law for use in Ontario, the package walks you through every major decision area with plain-language questions and clear yes/no choices:

• Your values and beliefs — what gives your life meaning and quality, and any religious, cultural, or spiritual beliefs that should shape your care (for example, wishes around last rites, natural death, or specific observances)
• Life-sustaining treatment — CPR, mechanical ventilation, feeding tubes, and dialysis, addressed scenario by scenario
• Pain and comfort care — palliative and hospice preferences, including whether comfort takes priority over curative treatment
• Dementia and cognitive decline — your wishes if you can no longer recognize loved ones or communicate
• Organ and tissue donation — your personal wishes, with guidance on registering at beadonor.ca
• Mental health treatment and any other instructions for your care team

The package includes step-by-step completion instructions, a signature and witness page, and a final checklist so nothing is missed. Once complete, you distribute copies to your doctor, family, and anyone who may speak for you.

Please note: this is not a Power of Attorney for Personal Care and does not appoint anyone to make decisions for you. A Living Will is a guidance document — it is not legally binding in Ontario the way a POA for Personal Care is, but it gives your caregivers and providers clear, morally persuasive direction about your wishes.
