Credentialing Tracker Google Sheets for Therapists, CAQH Re-Attestation, Recredentialing & License Expiry Spreadsheet, Excel Download
Stay on top of the credentialing clocks that keep running after you're paneled, in one calm spreadsheet.
Google Sheets / Excel only. This is the spreadsheet edition of the TherapistOps Credentialing Maintenance Tracker. Everything runs inside one Google Sheet (or Excel workbook), on desktop or the Google Sheets phone app. Nothing else to install.
Want the Notion version too? The full TherapistOps Credentialing Maintenance Tracker includes it. This listing is the spreadsheet only.
Log your last CAQH attestation and see the next one worked out for you (on a cycle you can adjust). Track each payer's recredentialing date, with overdue and due-soon flags. Get clear reminders before a license or malpractice policy expires. When something changes (a new address, a new malpractice carrier), a checklist shows every payer and directory you still need to update and logs when each one is done.
What's in the spreadsheet (10 tabs)
- Start Here: the setup steps, right inside the file
- Dashboard: next CAQH re-attestation and days left, credentials overdue / due soon, active payers, recredentialing overdue / due soon, open changes and updates still to do, plus credential and payer tables
- Credentials: CAQH, licenses, malpractice and anything else that expires; next due, days left and status calculate
- Payers & Directories: one row per payer, directory or registry; cycle used (with an optional per-payer override), next recredentialing, days left, status, updates logged and last update
- Changes: one row per change, with targets to update, updates logged, remaining and days open
- Update Log: each payer or directory update with date, method and confirmation; duplicates and unknown IDs are flagged
- Change Checklist: pick a change and see every target as TO DO or Done
- Renewal Calendar: the next 12 months of CAQH, license / policy and recredentialing dates, plus when reminder windows open
- Monthly Review: a live 10-point, 10-minute monthly checklist with a Done? column
- Lists: dropdown options plus three settings (CAQH cycle 120 days, recredentialing 36 months, reminder window 60 days by default)

Good to know
- Set up in about 5 minutes: open the file, delete the fictional SAMPLE rows, set your three cycles.
- The cycle lengths and reminder window are settings you can change. They're reminders you choose, not rules: set them to what CAQH and each payer tell you.
- Formulas are pre-filled through row 300 and shaded so you know not to type over them.
- Fictional sample data (5 credentials, 9 payers & directories, 2 changes, 13 logged updates) shows how everything works.
- Reference numbers are stored as the last 4 digits only.
- The images are previews rendered from the real spreadsheet with its fictional sample data. Menu wording in Google Sheets or Excel may differ slightly.
- You'll need your own Google account (free) or Microsoft Excel.
- This is a date and admin tracker only: it doesn't tell you what CAQH, a licensing board or any payer requires, and it isn't a substitute for any payer's requirements.
Digital download
This is a digital product: you'll download one ZIP file right after purchase. No physical item will be shipped. Because it's a digital file, returns and exchanges aren't accepted, but message me if anything doesn't open and I'll help.
Fine print
Not HIPAA-compliant storage. Not an EHR. This is an operational tool only, not clinical, legal or financial advice. Keep client data wherever your own compliance obligations require.
This tracker records dates and admin steps only. It does not tell you what CAQH, a licensing board, or any payer requires, and it is not a substitute for any payer's own requirements. Re-attestation and recredentialing cycles vary and can change, so verify current requirements directly with CAQH (CAQH ProView: https://proview.caqh.org/) and with each payer. The cycle lengths in this template are adjustable settings, not rules.
All sample data is fictional.
