Chiropractor Patient Tracker | Rebooking, Visit Packages & Care Plan Counts, Inquiry Pipeline | Notion Template + Google Sheets
Run the admin side of your chiropractic practice from one place, in Notion or Google Sheets. It also suits bodywork practitioners who sell visit packages.
Never miss an inquiry. Log each new inquiry with initials or a code. A reply flag shows Reply due, No reply – overdue (past your own reply target), or whether you replied on time, and a Next Step tells you what to do: reply, book the consult or first visit, or send forms.
Know who to rebook. Give each patient their own rebooking cadence (every 1, 2 or 6 weeks, whatever you choose). The tracker works out their rebook date from their last attended visit and flags them: Booked, On track, Rebook due soon or Rebook overdue.
Forms at a glance. Intake form, consent form and policies acknowledgment are simple checkboxes. A Forms Flag shows what's missing, and turns red when the patient already has a visit booked.
Visit packages and care plans that count themselves. Visits used and remaining, start and end / renewal dates, and alerts for low balances, used-up plans and upcoming renewals. A care plan here is just a visit count with dates: no clinical details.
Your visit types and balances. Visit types, lengths and prices in one price sheet that every visit looks up, and what each patient owes adds up automatically.
Two ways to use it (both included):
- Notion template: one click to Duplicate into your workspace. The Practice Hub has 5 linked databases (Inquiries, Patients, Visits, Visit Plans, Visit Types) with relations, rollups and formulas already set up. Views: Open Inquiries, All Inquiries, All Patients, Rebook Due, Forms Outstanding, Visit Log, Calendar, Upcoming, All Visit Plans, Needs Attention and Menu, plus five linked views on the hub page.
- Google Sheet: a Start Here tab with quick instructions, a Dashboard, and Inquiries, Patients, Visits, Visit Plans, Visit Types and Lists tabs, with dropdowns, reply and rebook flags, plan counts and balances.
What you get (instant digital download, one ZIP):

The reply target (24 hours), rebook due-soon window (7 days), renewal reminder (7 days) and low-balance alert (1 visit left) are settings you can change. They're your own choices, not standards or rules. Fictional sample data is included so you can see how everything works; delete it when you're ready.
Please note: this is an admin tracker only. It isn't designed to hold PHI (protected health information): don't enter diagnoses, treatment or care notes, or other health details. It doesn't give clinical, medical or billing advice.
Not HIPAA-compliant storage. Not an EHR. Not medical or legal advice. This is an organizational template only. Keep patient data wherever your own compliance obligations require.
Privacy & scope: This tracker is for admin steps only: inquiries, form checkboxes, bookings, visit packages, care-plan visit counts and payments. The intake form, consent form and policies acknowledgment are a checkbox only, and a care plan here is just a visit count with start and end dates. Keep the forms, care notes and any clinical or health details in the systems your obligations require, never in this tracker. The reply target, rebook window, renewal reminder and low-balance alert are your own adjustable settings, not standards or rules.
Not designed to hold PHI. Operational template only: no clinical, medical or billing advice. All sample data is fictional.
