How Families Can Use These Cards

• Write your baby’s name on the card, if desired
• Check the heart boxes that feel right for you
• Share the card with front-desk staff or your provider at appointments
• Print as many copies as needed

• Clinics are welcome to download, print, and share these cards as part of their bereavement or patient-support resources.

Printing Instructions

• Print pages 2–3 on cardstock (recommended)
• Enable double-sided printing
• Select “Flip on long edge” (landscape)
• Print at 100% scale (do not shrink to fit)
