Choose whether you want your child's first name only, first and last name, and if you wish to add their birthdate. Please specify how you would like the birthdate written (ie: January 10, 2020, or 01/10/2020 or whatever you prefer). There is NO additional cost for this option.

If you choose CUSTOM for the color, please provide the color you would like. The color will also be part of the personalization If you know the pantone color code for the color, it will be the best option. Otherwise we will send you an image of the closest one with your design for approval.
