✓ Blank editable Student Profile template
✓ Completed example to demonstrate how the profile can be used
✓ Space for a student photograph and key information
✓ Clear, one-page format designed for quick reference
✓ Editable Word format so it can be adapted for individual students
How can it be used?
The Student Profile can be completed using information gathered through student voice, parent/guardian voice, observations, transition information and existing school records.
It can then be used alongside the Student Support File and Student Support Plan, providing staff with a concise snapshot of the student and highlighting the approaches and supports that may help them participate successfully in school.
Because it is designed as an at-a-glance working profile, it can also be shared with relevant staff supporting the student, helping to promote a more consistent approach across teachers, SNAs and other members of the school team.
Suitable for post-primary SEN/AEN settings, autism classes and students receiving additional support.
