A fire safety officer or your insurer asks to see your fire records, and all you have is a loose pile of paper. This Fire Safety Log Book is a fillable PDF that keeps every routine fire safety check in one tidy A4 book: weekly alarm tests, emergency lighting, extinguishers, fire doors, drills, training, faults, servicing and your fire risk assessment actions. Type on screen and save, or print it and fill it in by hand.

For UK shops, offices, small workplaces, guest houses, B&Bs, pubs and cafés, and for landlords and managing agents who look after the common parts of HMOs and blocks of flats. Not for you if you need the fire risk assessment itself, automatic reminders, or records for a large site with many buildings.

Fillable PDF: type into it in the free Adobe Acrobat Reader, or in Chrome, Edge or Firefox on a computer, then save. Prefer paper? Print the PRINT file at A4.

What it does for you

  • Cover page with your premises and log book period, and a clickable contents page: click any sheet to jump to it (bookmarks too)
  • Start Here page: what the log book does, why you can rely on it and a 3-step plan, with the official sources
  • Premises details and key contacts: responsible person, fire risk assessor, alarm engineer and more
  • Checks at a glance: what to check, how often and who does it, filled in from GOV.UK guidance and easy to change
  • Daily checks for a month: escape routes, fire exits, alarm panel, emergency lights and signs
  • Weekly fire alarm test log with a call point register, so a different call point is tested each week, in turn
  • Emergency lighting: monthly tick grid for every light and exit sign, plus a test record for the annual full-duration test
  • Extinguishers and fire blankets (monthly checks and service dates) and fire door checks (closers, gaps, seals, signs, not wedged open)
  • Fire drill record (time to get out, roll call, what went wrong, actions) and staff fire training with refresher dates
  • False alarms and faults log, and servicing and inspection visits with certificate references
  • Fire risk assessment and action plan sheet: the details every Responsible Person in England and Wales must record since 1 October 2023
  • NEW in v5: premium cover and contents, Start Here page, bookmarks, a separate PRINT file and a filled-in EXAMPLE from a fictional guest house

How it works (3 steps)

  1. Open Fire-Safety-Log-Book-BLANK.pdf in Adobe Acrobat Reader (free) or your browser. Fill in the cover, Premises Details and Checks at a Glance (about 10 minutes), then save
  2. Do your checks: daily, weekly and monthly. Type each one in (press Tab to move to the next box, click a tick box to tick it) and save, or tick the printed sheet
  3. Record every drill, fault, service visit and your fire risk assessment actions. Start a fresh copy of the BLANK file each year and keep the old one

What's included

  • Fire-Safety-Log-Book-BLANK.pdf: the fillable log book, 16 A4 pages (cover, Start Here and 14 sheets)
  • Fire-Safety-Log-Book-EXAMPLE.pdf: the same book filled in for a fictional guest house, so you can see what good records look like
  • Fire-Safety-Log-Book-PRINT.pdf: a clean version without boxes, for printing and filling in by hand
  • Start-here.pdf: a short guide to filling in, saving and printing

Compatibility

  • Fillable on a computer in Adobe Acrobat Reader (free, recommended) and in Chrome, Edge and Firefox. We tested typing, ticking, saving and reopening in the PDF engines these browsers use
  • On a Mac, Adobe Acrobat Reader is recommended. Apple Preview and phone or tablet PDF apps handle forms differently, so fill in one page and save as a test first
  • Prints on A4: choose "Actual size" in the print settings
  • Standard PDF: no macros, no account, no subscription

Good to know

  • Dates are DD/MM/YYYY. Check intervals follow the GOV.UK fire safety risk assessment guides, for example a weekly alarm test from a different call point and a monthly emergency lighting check. Change them to match your own fire risk assessment and your installers
  • Save a copy after each session, and keep last year's book when you start a new one
  • A video tutorial is coming to our YouTube channel. Questions? Message PHKSheets
  • FAQ: Is this an official form? No. It is an independent template that follows GOV.UK guidance. The fire service does not require a particular log book.
  • FAQ: What if a sheet is full? Each sheet has a fixed number of rows (for example 22 weekly alarm tests). Save a fresh copy of the BLANK file, or print extra pages from the PRINT file.
  • FAQ: Does it work in Scotland and Northern Ireland? The records are useful anywhere in the UK. Section 156 and the Fire Safety (England) Regulations 2022 apply in England (Section 156 also in Wales); Scotland and Northern Ireland have their own fire safety law.

This log book is a record-keeping tool. It is not a fire risk assessment, legal advice or fire safety advice, and it is not produced or endorsed by any fire and rescue service or government body. Follow your own fire risk assessment, your installers' and manufacturers' instructions, and the law where your premises are. All names in the example are fictional.
