Wildlife Transport Form

When you're ready to move a wildlife casualty/orphan on from your Vet Practice to a Wildlife Rescue, a minimum of information NEEDS to travel WITH that animal, for the Rescue to use to provide ongoing care.

This form is the same one that UK Wildlife Transporters supplies to Vet Practices, to fill in & print out, to be handed to the Transporter of the animal, when they're picked up.
