Class Feedback Form - NOTION Template

A simple feedback form for teachers (whether for school, creatives teaching in person or online classes, etc.).

I chose a short feedback questionnaire because it's more likely to get responses than a drawn out one.

Feedback collected includes:

- which class the student is leaving feedback for

- where they heard about the class

- what they liked and would like to see improved

Optional information that the student may provide:

- screenshots of their project

- name and email - if they'd like a response from the teacher

The form, feedback sorted by date and class, and response requested tabs are already set up for you.

Developer Notes

- You can have the form email you when there are new responses. Instructions to do this are in the Credits and Help at the bottom of the page.
