Sensory Profile & Communication Passport — Editable Templates for Parents of Neurodivergent Children
Two editable templates designed to help parents of autistic, ADHD, and sensory-sensitive children share the right information with the right people — quickly and clearly.
What's included:
Sensory Profile — a structured overview of your child's sensory needs across seven areas: sound, touch, movement, vision, smell/taste, body awareness, and internal signals. For each area, record what your child finds difficult and what helps.
Communication Passport — a handout covering how your child communicates, signs of overwhelm and distress, what helps and what doesn't, favourite motivators, a do/don't quick-reference for adults, and emergency contacts.
Each template is included as an editable Word document (.docx) and a fillable PDF — four files in total.
DOCX: edit fully in Microsoft Word, Google Docs (free), or LibreOffice (free). PDF: blank fields throughout — type directly into it using any PDF reader, or print and fill in by hand.
Who it's for: Parents and carers of children with autism, ADHD, sensory processing disorder, or any additional need where briefing new adults clearly makes a real difference.
