As a 14D (144 DNA activated) experienced healer, Doc will move you through what is 'ailing' you and don't be surprised if the 'root' of it is completely different than what most professionals have taught. 

Print out and complete the form yourself and if you are helping someone who can't read or write, simply do your best to tell Doc what they are looking to heal, how long they have been ill and some basics like age (Birth Date) and location.
