A rhythm name should come after a description of what the trace shows. A consistent first look reduces guesswork and makes uncertainty visible.
1. Is the ventricular rhythm regular?
Compare the R–R intervals across the strip. A completely irregular ventricular rhythm is a useful clue, but regularity alone does not establish the diagnosis. Check whether any apparent irregularity is genuine rather than movement, poor electrode contact or another recording problem.
2. What is the ventricular rate?
Use a method appropriate to the recording speed and the rhythm. State whether the rate is slow, within an expected resting range or fast, then interpret it alongside the patient's presentation. A monitor value should be checked against the trace rather than accepted automatically.
3. Are consistent P waves present?
Look for organised atrial activity and ask whether each P wave has a consistent relationship with the following QRS complex. If discrete P waves are not visible, describe what you can see instead. Avoid forcing a label onto an unclear recording.
4. Is the QRS narrow or broad?
Measure rather than estimate where possible. QRS width helps organise the rhythm into a useful family, but it must be interpreted with the rate, atrial activity, timing and clinical context.
Put the description together
A useful summary might read: “Regular narrow-complex tachycardia at approximately 160 beats per minute; discrete P waves are not clearly visible on this strip.” That description is more transparent than jumping directly to a mechanism.
Clinical safety
This article is general professional education. A real patient who is unwell needs immediate clinical assessment and the appropriate emergency pathway. Do not delay care to complete a rhythm label, and work within your role and local guidance.
