MiCarePlan is a Maintaining Independence Care Plan, designed to support Older Adults remain living in their own home & community, allowing for their wishes, routines, needs and wants to be documented & communicated to their support persons. A catalyst for future proofing conversations, safety checks, division of tasks across support persons where required and ultimately, the prevention of unneccessary repetition across support perons, services and most importantly, the Older Adult. The daily planner allowing the Older Adult to live life on their terms, following their own will & preference.
