By enrolling in a 90-Minute Strategy Session with Mindful, Healthy, Balanced Health Coaching, I understand and agree to the following:

Nature of Coaching

Health and wellness coaching is an educational and supportive service designed to help clients explore nutrition, lifestyle habits, goals, challenges, and behavior change.

Coaching is not medical care and does not include diagnosis, treatment, prescribing, or management of medical conditions.

Medical Care

I understand that information discussed during my Strategy Session is not a substitute for care from my physician or another qualified healthcare professional.

I am responsible for consulting an appropriate healthcare professional regarding medical conditions, medications, symptoms, laboratory results, or changes to medical treatment.

Personal Responsibility

I understand that I am responsible for my own health decisions and for determining whether ideas, strategies, or recommendations discussed during coaching are appropriate for me.

Participation in coaching does not guarantee any particular health, weight, financial, or other outcome.

Confidentiality and Privacy

Information shared during the Strategy Session will be treated as private and confidential within the normal scope of the coaching relationship, except where disclosure may be required by law.

Because sessions are conducted electronically, I understand that no online communication platform can guarantee absolute privacy or security.

Virtual Session and Recording

The Strategy Session is conducted virtually through Google Meet. I am responsible for having suitable internet access, technology, and a reasonably private environment for the session.

The session may not be recorded, photographed, screen-recorded, or redistributed by either party without mutual consent.

Payment, Scheduling, and Rescheduling

The 90-Minute Strategy Session is a one-time coaching service priced at $147.

After purchase, I am responsible for scheduling my session using the scheduling link provided.

Any cancellation, rescheduling, or refund terms provided during purchase or scheduling are considered part of this agreement.

Scope of the Session

The Strategy Session is intended to provide focused coaching around the goals, questions, or challenges I bring to the session.

I understand that one session may help provide greater clarity, education, strategies, or next steps, but it is not intended to replace ongoing medical care or guarantee resolution of any particular concern.

Acknowledgment and Consent

By purchasing and participating in the 90-Minute Strategy Session, I confirm that I have read and understand this agreement, understand the scope and limitations of health and wellness coaching, voluntarily choose to participate, and accept responsibility for my own decisions and actions.
