About Me
Welcome! I am so glad you are here.
If you are a CNA, a nursing student, or a nurse new to the Skilled Nursing Facility (SNF) world, you already know that documentation is one of the most stressful parts of the job. It’s not just about "filling out forms"—it’s about telling the story of the high-quality care you provide, protecting your residents, and protecting your license.
Why I Created This Phrasebook
My name is Rochelle, and I am a Clinical Educator with over 24 years of nursing experience. I spent 17 years in high-acuity hospital settings—from ER and Neurology to Cardiothoracic Transplant—before moving into SNF education. I’ve seen "everything but the OR," and I know exactly what surveyors, providers, and families are looking for in your notes.
I created this guide to take the guesswork out of your shift. I want you to leave work knowing your charting is objective, professional, and survey-safe.
What’s Inside This Guide:
This isn't just a list of words. It is a complete toolkit designed for the "real world" of nursing:
- PointClickCare-Style Templates: Structured, fast, and easy to follow.
- Narrative Note Examples: Perfect for complex shifts and clinical rotations.
- CNA "Observe & Report" Prompts: Know exactly what to tell the nurse.
- SBAR Call Scripts: Feel confident when calling the MD or NP.
- Red Flags: Critical safety reminders for high-risk scenarios (Diabetes, SOB, Falls).
