These IEP parent and student input forms ask families and students for their ideas before a secondary IEP meeting, with every question in English and Spanish side by side.

Why I made this: In my experience, families who are more comfortable in Spanish often come to the IEP meeting without anyone asking for their input first, and students often hear their own goals for the first time at the table. These forms ask both of them ahead of time, in plain words, so their answers shape the draft.

What’s included:

• A case manager how to page

• A cover letter to send home (English and Spanish)

• A 3 page Parent Input Form (English and Spanish side by side)

• A 3 page Student Input Form (English and Spanish side by side)

• A case manager summary sheet for both forms

• Meeting and contact preference checkboxes, including interpreter and Spanish papers

How I use it: I send the letter and parent form home 2 to 3 weeks before the meeting and offer a phone call instead. Students fill out their form with me in class. I copy the main points onto the summary sheet and use them to draft present levels, transition goals, and accommodations.

Skills covered:

• Family input before an IEP meeting

• Student self advocacy and goal setting

• Planning for life after high school

• Language access for families

Who it is for: Secondary special education teachers, case managers, transition coordinators, and family liaisons working with Spanish speaking families.

What you download: IEP Parent Student Input Forms English Spanish.pdf (11 pages, US Letter, print ready) plus an editable Word version you can type into.

How this is different: The Transition Family Map, Ages 14 to 21 is an "all about me" family profile. These are short, meeting specific input forms for one upcoming IEP meeting, with a matching student form and a case manager summary sheet.

Part of the Transition Cliff family series.
