ECM Questionnaire — Children's Version (5–11)

This questionnaire is designed to complement work with the Emotion Communication Map (ECM) but may be used as a stand-alone resource.

It is completed with children aged 5 to 11, exploring how a child experiences, expresses, and communicates their emotions in language that is accessible and age-appropriate — forming a neuroaffirming baseline that supports the ECM process.

It is not a diagnostic assessment. All responses are child-led and can be completed with support from a clinician, educator, or trusted adult.

The questionnaire covers eight sections:

• Feelings inside my body
• Things that bring big feelings
• How my feelings show
• Do people understand me?
• Understanding other people
• Words for feelings
• Things I have tried

When completed, the questionnaire generates a personalised profile which can be printed or saved as a PDF for use in clinic, school, or at home.
