MinistryForge Mini Tools: Church Decision & Follow-Up Pack

A spiritual decision made during a church service, revival, youth activity, VBS, outreach event, or personal conversation can be an important moment in someone’s life. But the ministry shouldn’t end when the service is over.

The MinistryForge Church Decision & Follow-Up Pack gives pastors and church leaders a simple system for recording spiritual decisions and making sure meaningful follow-up actually happens.

This practical 3-page resource helps you capture important information while the conversation is still fresh, assign responsibility for follow-up, track subsequent contacts, and encourage the individual toward appropriate spiritual next steps.

The Pack Includes:

1. Spiritual Decision Record

Record the individual’s information and the decision or response that was made, including salvation, assurance, rededication, baptism, surrender to ministry, or another spiritual decision. There is also room for conversation notes, salvation or assurance details, immediate next steps, and the person responsible for follow-up.

2. Decision Follow-Up Planner

Keep follow-up from becoming an intention that never happens. Track contacts, who made them, what happened, questions or concerns that surfaced, follow-up progress, and the next action that needs to be taken.

3. Baptism & Discipleship Follow-Up

Help someone move beyond a moment of decision toward continued spiritual growth. Track salvation follow-up, baptism, discipleship, church connection, ministry opportunities, and future follow-up needs.

The goal is simple:

Record the decision. Follow up intentionally. Encourage the next step.

This pack can be especially useful for decisions made during regular church services, revival meetings, VBS, camps, youth activities, outreach events, special services, or one-on-one ministry conversations.
