A dated record of what you and your prescriber decided, kept in one file you can bring to the next visit. It contains no medical guidance of any kind.

One row per dose, in date order, with the dose typed exactly as it appears on your prescription. The column is free text on purpose, because the file has no opinion about what belongs in it. Weight and tape go on their own tab whenever you take them, side-effect notes are written in your own words with a date, and appointments record who, when, and what to ask.

THE PART NOBODY ELSE BUILDS

The Dashboard shows the date each injection site was last logged and names the one used least recently. That is a read of your own rows and nothing more; what you do with it is between you and your prescriber. The unit is one cell at the top of the Weight tab, and the column headers, the Dashboard note and the chart legend all read that cell, so switching it changes the file rather than half of it. Figures already typed are never converted behind your back.

Works in Microsoft Excel 2016 and later, and in Google Sheets. This is a log. It gives no dosing guidance, makes no medical or outcome claims, and decides nothing for you. Speak to your prescriber about anything it records. Digital download, all sales final.
